Notification of Infringement

- Identification of the material that is claimed to be infringing or to be the subject of infringing activity and that is to be removed or access to which is to be disabled, including information reasonably sufficient to allow us to locate the material.

- A statement that you have a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
- A statement that the information in the notification is accurate, and under penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
Procedure to Follow
Upon receipt of a valid infringement notification, Illinois PDF Forms will swiftly take the necessary actions to remove or disable access to the material claimed to be infringing. Additionally, we take steps to notify the user who posted the alleged infringing content, providing them with an opportunity to issue a counter-notification.

Counter-Notification
Individuals who have received a notice of infringement may submit a counter-notification under sections 512(g)(2) and (3) of the DMCA. To be effective, a counter-notification must be a written communication that includes substantially the following:
- Your physical or electronic signature.
- Identification of the material that has been removed or to which access has been disabled and the location at which the material appeared before it was removed or access to it was disabled.
- A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification of the material to be removed or disabled.
- Your name, address, and telephone number, and a statement that you consent to the jurisdiction of the federal court for the district in which your address is located, or if your address is outside of the United States, for any district in which Illinois PDF Forms may be found, and that you will accept service of process from the person who provided notification under subsection (c)(1)(C) or an agent of such person.
Following the submission of a valid counter-notification, Illinois PDF Forms will generally restore the material in question, unless we first receive notice from the original reporting party that they have filed an action seeking a court order to restrain the alleged infringer from engaging in the infringing activity.
